advocate dedicated to the “YourChoice” plan. Call the
pharmacist at 941.748.4501, ext. 6406. Meet the
YourChoice Pharmacy Advocate.

The pharmacy plan is administered by Prescription
Solutions. They have a customer service center and can
answer all types of questions about current and past
prescriptions. Call them at 800.797.9791.

Preferred Retail And Mail Order Pharmacies
These retail pharmacies offer the lowest copay for
members.
  • Generics: No co-pay (free of charge to the
    member)
  • Preferred Brands: Minimum of $15 copay for or
    25% of the approved cost
  • Non-Preferred Brands: Minimum of $40 copay
    for  or 45% of the approved cost
  • Specialty drugs: Minimum of 25%
